Patient Information


We welcome new patients, and we love your referrals.  Our reception area is clean, bright and cozy. We find our patients saying they’d love to just stay and chat.  We are okay with that.  We love to know you feel relaxed and comfortable.  We respect your time and try to keep a “no waiting” policy through flexible scheduling and prompt appointments. If you would like an appointment please send us an email with your preferred days and times. We do not keep a full week of appointments, because we like to keep a flexible schedule to hold space for last minute appointments, emergencies, and life’s little uh-ohs.


We do not participate in a dental insurance PPO or managed care network, however we will gladly file your insurance claim and work with you to maximize your dental benefits.  We are very respectful of your finances and will work with you to ensure that your health care needs are addressed.  We accept cash, checks, debit and credit cards. We recognize that dental care can be expensive, so try to find ways to make flexible payment plans possible.

The patient registration forms are listed below.  You may print these out, complete them and bring them to your appointment.  If would you prefer, we can send you a link to complete our mobile friendly forms that will electronically update your chart. Of course, if you’d rather complete them while in our office, please be our guest. 

We have also provided instruction forms for post-procedure home care.
